Preparing for Your First Visit

We ask that new patients please arrive 15 minutes prior to their first appointment. Please bring the following items with you when you come for your appointment:

• Patient Information forms (available here »)
• List of current medications
• A few pair of shoes
  that you typically wear
• A government-issued photo ID
• Insurance card

All minors (under 18 years of age) must be accompanied by a parent or guardian.
